At Crown on Mane, we want clients to feel prepared before scheduling a medical wig or cranial prosthesis consultation. One of the most important steps is obtaining the correct documentation from your doctor or licensed medical provider before your first appointment.
This documentation may be needed for insurance purposes and helps confirm that your hair loss is medically related.
Before You Book Your First Medical Wig Consultation
Before scheduling your first cranial prosthesis or medical wig consultation with Crown on Mane, please obtain the following from your doctor or licensed medical provider:
- A prescription for a cranial prosthesis
- A letter of medical necessity
These documents are especially important if you plan to seek insurance reimbursement or submit a claim to your insurance company.
Please do not upload or submit prescriptions, diagnosis details, medical records, insurance ID numbers, Social Security numbers, or sensitive medical information through the public booking form. If documentation is needed, Crown on Mane will provide secure instructions after your appointment is scheduled.
Crown on Mane does not diagnose medical conditions, prescribe, treat scalp disorders, or guarantee insurance approval, coverage, or reimbursement.
What Is a Medical Wig?
A medical wig is a wig or hair replacement piece used because of medically related hair loss. It may also be referred to as a cranial prosthesis, cranial hair prosthesis, or hair prosthesis.
Medical wigs may be used by clients experiencing hair loss related to:
- Alopecia
- Chemotherapy or radiation treatment
- Surgery
- Medication-related hair loss
- Autoimmune conditions
- Burns or scalp trauma
- Medical thinning or shedding
- Other health-related hair loss
The term medical wig does not mean that the wig itself treats hair loss. It means the wig or prosthesis is being used to help restore coverage, comfort, and confidence when hair loss is related to a medical need.
What Is a Cranial Prosthesis Prescription?
A cranial prosthesis prescription is a written order from your doctor or licensed medical provider stating that you need a cranial prosthesis because of medically related hair loss.
The exact wording can matter. Some insurance companies may not recognize the word “wig” for reimbursement purposes. They may require the wording “cranial prosthesis” or “cranial hair prosthesis.”
A prescription may include details such as:
- Your name
- The provider’s name and contact information
- The medical reason for hair loss
- The recommended item, such as “cranial prosthesis”
- Cranial prosthesis measurements, if your doctor or provider includes them
- Diagnosis information, if required by insurance
- Provider signature and date
Sometimes a doctor or licensed medical provider may provide measurements for the cranial prosthesis. If measurements are included with your prescription or documentation, please bring a copy to your consultation. Crown on Mane may also take or confirm measurements during the consultation or fitting process to help with sizing and comfort.
Because insurance requirements vary, clients should contact their insurance company to ask what wording and documentation are required.
What Is a Letter of Medical Necessity?
A letter of medical necessity is a letter from your doctor or licensed medical provider explaining why a cranial prosthesis is medically necessary.
This letter may help support an insurance claim or reimbursement request. It may explain that hair loss is related to a medical diagnosis, treatment, surgery, medication, or another health-related condition.
A letter of medical necessity may include:
- Your name
- The provider’s name and office information
- The medical condition or treatment related to hair loss
- Why a cranial prosthesis is medically necessary
- The expected duration of need, if applicable
- Provider signature and date
Crown on Mane cannot write a letter of medical necessity for you. This must come from your licensed medical provider.
Why Documentation Matters
Documentation matters because medical wig and cranial prosthesis coverage depends on your insurance plan and policy requirements.
Some insurance plans may offer partial or full reimbursement. Others may deny coverage or consider the item cosmetic. Requirements may vary by insurance company, employer plan, diagnosis, and state-specific rules.
Having a prescription and letter of medical necessity before your consultation helps you be more prepared if you plan to submit a claim.
It may also help clarify whether you are seeking a medical cranial prosthesis rather than a cosmetic wig.
Questions to Ask Your Insurance Company
Before your consultation, it may be helpful to call your insurance company and ask specific questions.
You may want to ask:
- Does my plan cover a cranial prosthesis?
- Does my plan use the term cranial prosthesis, hair prosthesis, cranial hair prosthesis, or wig?
- Is a prescription required?
- Is a letter of medical necessity required?
- Does the prescription need specific wording?
- Are cranial prosthesis measurements required or recommended?
- Is there a specific diagnosis code or procedure code required?
- Is prior authorization required?
- Do I need to purchase from an in-network provider?
- Can I submit for out-of-network reimbursement?
- What documents are required with the claim?
- Is there a maximum reimbursement amount?
- How often will the plan cover a cranial prosthesis?
- Where should the claim be submitted?
- What claim form is required?
Write down the representative’s name, the date of the call, and the reference number for the conversation if one is provided.
Insurance Coverage Is Not Guaranteed
Crown on Mane can help you understand general documentation questions, but we cannot guarantee that your insurance will approve or reimburse your purchase.
Insurance decisions are made by your insurance company. Coverage may depend on:
- Your specific plan
- Your diagnosis
- Your provider documentation
- Your deductible
- Your out-of-network benefits
- Your policy exclusions
- Whether prior authorization is required
- Whether the item is considered medically necessary under your plan
Even with a prescription and letter of medical necessity, reimbursement is not guaranteed.
What to Bring to Your Consultation
If you have printed copies available, please bring:
- Your cranial prosthesis prescription
- Your letter of medical necessity
- Any cranial prosthesis measurements provided by your doctor or licensed medical provider
- Any insurance instructions you received
- Any claim forms your insurance company requires
- Notes from your insurance call
- Photos of hairstyles you like, if helpful
- Questions you want to ask during the consultation
Please do not bring original documents that you cannot replace unless necessary. It is usually best to keep copies for your own records.

Please do not include the following in the public booking request:
